Nothing more serious than badly needing to pee or take a dump. Last time we played MK8 online together I had to pee on two occasions. Thankfully if you don't choose a course quickly and let the time run out it just about gives you enough time to run to the bog for a quick pee.
The absolute worst was playing Dark Souls and leaving your character at a spot with no danger of getting killed for just a few moments to go to the toilet and then return to find out you'd been invaded and mauled.

LOL yes the souls bathroom break is terrifying.
But the worstd worst thing are games with unpausable cutscenes and as soon as the most important cutscene starts knock on the door. Fuuuuu.

I got invaded just before it was dinnertime. I hid, and 30 minutes later when I came back the invader was still trying to find me.

Oh God I hate that, you're playing some game with a story and a cutscene starts that you have to listen to and someone walks in and starts yammering nonsense at you while you're trying to concentrate.

I think the worst for me has been the cat taking a shit next door and the stench just grows more and more powerful. So you have to pull your tshirt over your nose and mouth, sitting there trying to play a match and the stench is so bad you can feel your eyes start to water.
Also I have had random butt pain on two occasions now. I don't think the people you're playing against understand that it feels like you are sitting on ragged rocks.
