What's the deal with "extreme" difficulty?

i think fighting games are fair play so long as they are well balanced. let people memorise dozen button combos if they think that will put them in good stead, you still have a fighting chance by sticking to your guns (even if you use basic attacks only) and knowing to apply them well.
And of course button mashing will always beat any 'pro' fighting game player.
