If his internet is slowed then presumably he doesn't pay anything extra ones he gets over; the internet just becomes 56k speed or something around that. And it is monthly, but that doesn't mean it'll be fast again today as it goes by when the contract was signed rather than when the month officially begins and ends. Mine resets on the 10th each month for example.
